Help and Support

We never decide anything about your care and support without your full involvement; needing support does not mean giving up your independence. The first thing we will do is discuss your lifestyle, ideals and feelings to come up with a plan together that will give you the confidence and independence you deserve. Some of the general support we can provide;

  • Personal care and assisting with dressing and personal appearance
  • Help and support with daily activities, mobility and exercise
  • Medication prompting and monitoring
  • Meal preparation and cooking regular nutritious meals
  • Driving to medical appointments, meetings and social events

Emotional and Extra Support

We believe emotional support is just as important as physical support. We go above and beyond, making time for companionship if you feel that is what you need, acting as a link to the outside world or just a listening ear. Some of the extra support we can provide;

  • Keeping you in touch with the world by reading newspapers, magazines and books
  • Companionship while watching television and DVDs or listening to music
  • Helping care for pets and plants
  • Maintaining a safe, secure home environment
  • Home Management including the organisation of property maintenance, electrical and repairs, and purchasing household items

Mental Health Services

Our community support services include specific services for domiciliary care, supported living, learning disabilities, autism.
Care Solutions Healthcare will deliver this through the help of our outreach team that engage and support individuals with a dual diagnosis living in their own tenancies. We will also provide move on and rehabilitation services for people with a range of needs, including support for individuals with severe and enduring mental health needs, as well as older adults with functional and organic mental health problems.
Our supported housing services range in type of accommodation and include sheltered housing, a 24 hour rehabilitation hostel, crisis beds to prevent hospital admission, residential rehabilitation services; intensive support services for individuals resettled from hospital and supported housing schemes.

Care Solutions Healthcare is committed to working in line with the National Service Framework (NSF) for Mental Health. Care Solutions Healthcare sees how important it is for service users to have meaningful lives, independent living skills and good self-esteem. Our support focuses on recovery and our services work towards rehabilitation for service users, developing skills so that people can take greater control of their lives. We seek to support the person to make progress towards recovery through recognising and building on their strengths, coping abilities, social networks and support systems.

We actively work with each of our service users so they develop the necessary skills to move-on successfully to less supported or independent accommodation with reduced hours of support. We achieve this through thorough assessment, support planning and review procedures, including Wellness Recovery Action Planning (WRAP), partnership working with housing providers and creative approaches to setting up good quality move-on options.

All Care Solutions Healthcare mental health services promote an all round approach to rehabilitation and work according to the principles of social inclusion and the Recovery Model. We work towards individual outcomes, with appropriate support in areas such as training, education, employment, emotional and spiritual well-being, maximising the use of existing community facilities and the promotion of independence to enable individuals to move on and take control over their lives. We work closely with the families and friends of service users to build informal support networks and supportive relationships.

Our staff members working in services for people with mental health needs are given training on the Recovery Model in their induction and as necessary during their employment to ensure a positive understanding of recovery. The effective promotion of the recovery approach and use of WRAPs also needs commitment from our service users to engage in a style of action that is service user directed and works by establishing and putting into practice coping strategies, crisis planning, collaborative working and a realistic and effective approach to recovery. This is a model of “co-production” which pools the resources of the service user, their support networks, our services and partner agencies to produce the best possible outcomes for the service user.


Learning Disability Services

Care Solutions Healthcare will provide a tailored flexible, efficient and person centred service to meet the different needs of our service users. Our services will include those designed specifically for individuals based on their assessed needs as well as larger contracts which offer flexibility and choice as well as promoting the customers’ independence and personal dignity.

We have developed a range of services including intensive support for people with challenging behaviours, dual diagnosis, autism specific services, services for people with additional physical and sensory needs and community support for people who are more independent. We work closely with families, carers and other stakeholders to provide co-ordinated services for individuals. There is a high level of active service user participation with regular forums for service users with learning disabilities.

Across our services we highlight the importance of people being supported and encouraged to participate in a wide range of activities, and staff ensure that people enjoy full and active lifestyles. This helps to increase social inclusion and to make sure that people with learning disabilities have full access to the community resources, events and activities available. This in turn improves health and well-being and gives people the opportunity to strengthen their social networks and develop their confidence in social situations.

The key part of our approach to Person Centered Planning is that the individual is at the center of their plan, and is in control of their plan and what it contains. We make sure that service users and carers are fully involved in the development of Person Centered Plans.

All our employees working in services for people with learning disabilities are trained in Person Centred Planning: working in person centred ways, promoting rights and choices and developing Person Centred Plans (ELP, Path and MAP) with our service users. They are also trained to meet specialist communication needs so they can facilitate Person Centred Planning with service users who have little or no verbal communication, using communication techniques such as British Sign Language (BSL), Makaton and Picture Exchange Communication Systems (PECS). This ensures that all service users are able to plan around their individual goals and preferences.

We believe that everyone has the potential to make decisions about their lives, and that Support Workers must be skilled in helping them to do this, regardless of a service user’s ability to communicate verbally. Care Solutions Healthcare expects all staff to ‘listen’ carefully at all times to service users and take appropriate action based upon their wishes, intentions and dreams

Autistic Spectrum Disorders

Care Solutions Healthcare is an experienced provider of services for people who are on the Autistic Spectrum. We are registered with the National Autistic Society (NAS) and have developed a number of innovative, high quality services that enable people with autism to lead happy and fulfilling lives. All of our services are registered with the CQC (Care Quality Commission, formerly CSCI – Commission for Social Care Inspection) and are delivered in a person centred way designed to promote independence and personal development.

The services that we deliver to people with autism are designed around individual needs. For this reason no two services look the same and we are proud of the way we have been able to tailor support packages and the living environment around the specific requirements of our service users (and their families).

Understanding The Individual & Their Support Needs

Care Solutions Healthcare knows the importance of understanding the personalities and preferences of the people that we support. When we are putting a support package together we feel it is essential to work closely with the people that know the person best. Often this is the person’s own family but can equally include carers, professionals and friends. The professionals we often work with when delivering autism specific services include Speech and Language Therapists, Occupational Therapists, GP’s, Behavioural Specialists, Social Workers, Psychologists and Psychiatrists. We feel it is important to recognise and make use of the unique skills that each and every person can bring as part of a team approach.
We are also aware that to understand the individual person we need to understand how the sensory world and their environment affect their wellbeing and happiness. This is a key part of our work in autism services.

Transition & Person Centred Planning

Coping with change is difficult for most people but especially for people who are on the autistic spectrum. When we are supporting people with autism to make a significant move in their life we place a great deal of importance on detailed planning to achieve a high quality, smooth transition. We have experience of supporting people to make the transition from living within the family home, residential school placements and ‘out of county’ placements into a new support setting.
Person Centred Planning (PCP) plays a key role in ensuring that each person’s support needs, dreams and goals are being met. Within autism services we feel this is a particularly important process as services users often have difficulty in clearly expressing their preferences and wishes. We use a range of PCP tools and specialist communication techniques so that all individuals have a personalised plan and service in place whatever their level of communication and need.

Communication

At Care Solutions Healthcare we deliver our services to people with autism in a way that recognises and respects that people communicate in different ways. We understand the importance of meeting individual communication needs and we train our staff to do so. Within our services it is common for communication aids such as Makaton, British Sign Language, PECS (Picture Exchange Communication Systems), Objects of Reference, TEACCH schedules and communication passports to be used to enable our support staff to communicate with our service users positively and effectively. This ensures that they provide the support the service users need and want.

Recruiting & Training The Staff Team

Care Solutions Healthcare provides opportunities for service users and their families to get involved in the recruitment of the staff team to make sure we recruit the right people. We employ people who have the positive values, personalities, skills and knowledge to provide effective support to people with autism. Once in post we recognise that all staff needs on-going training to increase their knowledge and understanding of autism. All staff undertakes the NVQ (National Vocational Qualification) in Health and Social Care and the LDQ (Learning Disability Qualification) and also access autism specific training and personal development programmes on an ongoing basis

Personalisation

The Government’s Green Paper "Shaping the Future of Care Together" states that its vision is for ‘a system where people will be able to live their lives in the way they want, supported by the services that they choose’, and that services ‘should respond to local conditions’ Put simply, person-centred planning is a way of discovering what people want, the support they need and how they can get it. It is evidence-based practice that assists people in leading an independent and inclusive life. Person-centred planning is both an empowering philosophy and a set of tools for change, at an individual, a team and an organisational level. It shifts power from professionals to people who use services.

So how are we going to do it? Focusing on the person’s strengths, passions, interests and the things that others like and admire about them provides a tremendous starting point for a co-productive relationship. Care Solutions Healthcare is committed to the principles of the personalisation agenda as set out in the Government’s papers: ‘Our Health, Our Care, Our Say, ‘Valuing People’, ‘Valuing People Now’ and ‘Putting People First’.

We understand that the adult social care system is changing and we support the thinking that:

  • People have a right to self-determination
  • People have a right to services personally tailored to their needs
  • People have the right to shape and buy in their own services
  • There should be strong self-advocacy and family and carer support and involvement

We believe that individualised, person centred planning is the foundation for truly personalised services. It empowers service users to take control of their lives – they tell us what they want from their support and how their goals are going to be achieved. Self-directed support adds to this, putting people in control of their funding and of the decisions on how best to meet their needs.

Care Solutions Healthcare has embraced alternative funding models as put forward by the In Control project (self-directed support, Individual Budgets), which enable people to exercise more choice and control over their support and we already support a number of service users with Direct Payments and ILF funding.
Any of our service users assessed as eligible to receive a Direct Payment are supported to access local advice and brokerage services to buy in the support of their choice.
We work with service users who have Individual Budgets and who choose to be supported by us, to make the most of their allocated budget, and to meet their needs in person centred and creative ways. We believe it is vital to involve family, carers and advocates in this process to ensure people are really able to tell us what they want and what works best for them.
We welcome enquiries from service users and their families or carers who are interested in receiving support from our agency using their individual budget. We will work with you to develop the best support package possible to meet individual needs.

We ask that our care team are always smartly dressed when they visit our clients, we don't insist on uniforms, we know that our clients want a discrete professional service that allows them to maintain their lifestyle and independence. At Care Solutions Healthcare we believe that support is emotional as much as physical and understand the need for more than just assistance with daily tasks. Because we create a safe and happy work environment for our staff there will be no rushing away, we allow for companionship time in your plan if you feel that is what you need, and assistance with day to day concerns, or just a listening ear.

We are continuously looking to improve our service to our clients as well to our care team. This culture of improvement is driven from the top of our business and is embraced by the whole team. We visit our clients regularly to make sure their care and support plan meets their needs and also to make sure that their care is delivered to the highest standard. Our senior team understand the importance of regular review and feedback, we send out questionnaires to our clients and their family for their views on how well we are delivering our service. These feedback forms are reviewed by the senior team to make sure we maintain our standards.
